Located at 6022 Harvey Street , just south of the Lakes Mall. The clinic is easily accessed from U.S. 31 by taking either the Sternberg or Pontaluna exits.
Hours of operation : Monday- Friday 8:00-5:00. Appointments outside of these hours are accommodated as needed.
